About this file
This is a Texas Family Code Schedule document required by the State of Texas for Solicitation No. 601310000099387, establishing eligibility requirements for respondents seeking state grants, loans, or contract payments. The schedule serves as a certification mechanism under Texas Family Code Section 231.006, relating to child support obligations. Respondents must complete the form by providing names and Social Security numbers for any individual or business entity holding at least twenty-five percent ownership interest in the submitting business entity. The document explicitly states that failure to sign and return this schedule may result in the response being deemed non-responsive, making completion mandatory for all applicable submissions.
The schedule functions as a compliance verification tool whereby respondents certify their eligibility to receive specified payments and acknowledge that the contract may be terminated and payment withheld if the certification is inaccurate. Social Security number disclosure is required under Texas Family Code Sections 231.006(c) and 231.302(c)(2) and will be used to identify individuals who may owe child support obligations. The information collected will be maintained confidentially to the fullest extent permitted under Section 231.302(e) of the Texas Family Code. Respondents who do not meet the ownership criteria may check an applicable box and return the schedule accordingly. The document must be signed by an authorized representative and include the signatory's title.
