About this file
This is an Addendum #1 to RFQ #717-26-731 issued by Texas State University (TSU) Procurement Services for Cost Estimating Services. The addendum addresses procedural and submission requirements clarifications through a questions and answers format. Key submission requirements include the Execution of Offer form, Form 1295, and TSU Terms and Conditions, which must be organized separately in the table of contents with individual tabs inserted after responses to Section 3 of the Qualifications. An Architect/Engineer (A/E) is required and must be registered in the state of Texas.
The addendum clarifies that several initially questioned forms are not required, including the HUB Plan, HUB Vendor Agreement, Conflicts Disclosure Statement, Conflict of Interest Questionnaire, W9, Felony Conviction Notification, and TSU Terms and Conditions (though the latter was later confirmed as required). For firms providing preconstruction cost estimating services before a General Contractor is engaged, architect information is sufficient and may be provided in place of General Contractor information in Criteria Four (specifically section 3.4.1). Bidders must acknowledge receipt of this amendment by signing and including the signature page in their response submissions.

Questions and Answers RFQ # 717-26-731 Cost Estimating Services
1. Can you please confirm that the following forms are required for our submission: Execution of Offer, HUB Plan, TSU Vendor Agreement, Conflicts Disclosure Statement, Conflict of Interest Questionnaire, W9, 1295 Form, Felony Conviction Notification, and TSU Terms and Conditions.
Response: Execution of Offer, form 1295 and Terms & Conditions are required.
2. If forms are required, how do you want the forms packaged for the submission?
Response: Each item shall be included separately in the table of contents and identified by a separate tab inserted after responses to Section 3 of the Qualifications.
3. Due to recent HUB status changes by the Acting Texas Comptroller, and it was not mentioned in the RFQ, is the HUB Plan valid in our submission and required?
Response: No HUB Plan is required.
4. Is an A/E required, and if so, do they need to be registered in the state of Texas?
Response: Yes and yes
5. Do we need to include General Contractor information in Criteria Four since (Specifically 3.4.1) our services are typically in the preconstruction stage before a General Contractor comes on board? Would providing the architect's information be sufficient in place of the General Contractor?
Response: No, the architect’s information is sufficient.
