About this file
This is a Texas Family Code Schedule certification document associated with Solicitation No. 601440000051803 from the State of Texas. The schedule serves as a compliance verification tool for respondents to attest to their eligibility to receive state grants, loans, or payments on state contracts under Texas Family Code Section 231.006, which addresses child support obligation requirements. Respondents are required to complete the certification by providing names and Social Security numbers for all individuals or business entities holding at least twenty-five percent ownership interest in the submitting business entity. The document mandates that this completed and signed schedule be returned with the response submission, with failure to do so potentially resulting in the response being deemed non-responsive.
The document emphasizes that Social Security numbers are required under Texas Family Code Sections 231.006(c) and 231.302(c)(2) to identify persons who may owe child support obligations. The collected information will be kept confidential to the fullest extent permitted under Texas Family Code Section 231.302(e). Respondents who do not meet the ownership criteria specified may check an applicable box to indicate that the schedule is not applicable to their submission. This certification is a mandatory component of bid submissions and serves as a prerequisite for contract award and payment authorization, with the state retaining the right to terminate the contract and withhold payment if the certification is found to be inaccurate.
