About this file
This is a Texas Family Code Schedule form associated with Solicitation No. 601330000045858, issued by the State of Texas to establish eligibility requirements for contractors receiving state payments. The schedule requires respondents to certify their eligibility to receive state grants, loans, or contract payments under Texas Family Code Section 231.006, which pertains to child support obligations. Respondents must provide the names and Social Security numbers of all individuals or business entities holding at least twenty-five percent ownership interest in the submitting organization. This certification is mandatory, and any inaccuracy may result in contract termination and payment withholding. The form must be completed, signed, and returned with the response submission, with failure to do so potentially rendering the response non-responsive.
The schedule serves as a compliance verification tool rather than a pricing or contract terms document. Respondents meeting the ownership criteria must list all required owners with their corresponding Social Security numbers, while those not meeting the ownership threshold may indicate non-applicability by checking the provided box and signing the form. Per the Federal Privacy Act Notice, disclosure of Social Security numbers is required by law and will be used to identify individuals with outstanding child support obligations. The Social Security information will be maintained confidentially in accordance with Texas Family Code provisions. This schedule must accompany all bid responses and represents a mandatory compliance requirement for contract eligibility.
