About this file

This document is a Texas Family Code Schedule for Solicitation No. 601330000044374, which addresses ineligibility requirements for state grants, loans, and contracts related to child support obligations. The document requires respondents to disclose ownership information, specifically identifying individuals or entities holding at least a 25% ownership interest in the business submitting a response, by providing names and social security numbers.

The schedule is part of a compliance mechanism under Texas Family Code Section 231.006, which ensures that entities seeking state payments do not have outstanding child support obligations. Respondents must either complete the ownership information section or check a box indicating the schedule is not applicable. The document includes a Federal Privacy Act notice explaining that social security numbers will be used to identify potential child support debtors and will be kept confidential to the extent allowed by law. Failure to sign and return this schedule may result in the response being considered non-responsive.
