About this file

This document is an Attachment 2 Bid Submission Checklist for a government solicitation, providing guidance for potential bidders on preparing a responsive bid. The checklist outlines several required forms that bidders must complete, including Form 1 (Execution of Bid), Form 2 (Cost Sheet), Form 3 (Financial Questionnaire), and Form 4 (Substitute W-9 and Direct Deposit Form, if applicable). The document emphasizes that while the checklist is not a mandatory attachment, bidders are responsible for thoroughly reviewing all solicitation requirements and ensuring a complete bid submission.

The checklist also indicates that bidders should prepare Public Information Act redacted copies if necessary. The document is designed to assist bidders in organizing their proposal materials and ensuring they meet all necessary submission requirements. No specific details about the project's scope, budget, or specific procurement details are provided in this particular document, which serves primarily as an administrative guide for bid preparation.
