About this file

This document is a Form 1 - Execution of Bid for Invitation for Bid (IFB) 3202500180 issued by Texas Workforce Commission (TWC) for a Current Procedural Terminology (CPT) Data File License. The bid document outlines the requirements for potential bidders to provide a CPT data file license, with bidders required to complete and sign the form acknowledging receipt of all TWC terms and conditions. The form includes sections for bidders to disclose any exceptions to the terms, provide company and contact information, and declare any subcontractor interests.

The bid form includes provisions for bidders to notify TWC of changes in financial condition, ownership, or control, and to provide information about potential litigation or adverse actions that could impact contract performance. The document also offers opportunities for bidders to claim various statutory preferences, including preferences for Texas-based businesses, service-disabled veteran-owned businesses, products produced in Texas or the USA, environmentally sensitive products, and other special categories. Bidders must have an Employer Identification Number (EIN) and a Texas Identification Number (TIN) prior to contract award, with detailed instructions provided on how to obtain these identifications.
