About this file
This document is a Warranty and Complaint Resolution Form for a contract opportunity issued by the Texas Health and Human Services Commission (HHSC) for the procurement of food products for the Lufkin State Supported Living Center (SSLC). The contract term is from September 1, 2024 through August 31, 2025, with possible renewals. The contractor must guarantee that the products comply with all contractual requirements and that they will resolve any complaints received on local groceries in a timely manner, within 90 days. The contractor is responsible for associated costs, including storage, HHSC employee hours, samples, shipping, and lab testing. If the delivered product is found to be non-specification in part, the entire shipment will be reimbursed or replaced by the vendor. Unacceptable response, resolution, or failure to meet required time frames will result in the vendor being tracked and potentially having future HHSC food awards discontinued.
