DIRECTIONS

NOTE TO RFR DEVELOPERS – DELETE THIS TAB and EVALUTION CRITERIA TAB PRIOR TO POSTING WITH BID:
Environmental Practices Form:

The Environmental Practices Form (and should be referred to in the RFR as such) tab is provided here as a guidance template for RFR developers, and requires editing prior to including in an RFR or RFQ. All Statewide Contract bids should include this form, and departments are also encouraged to do so.

Each sourcing team should edit the form as applicable for the products/services bid. If entries in the template are not applicable, bid developers should remove and only keep those entries that make sense for the specific bid (e.g., packaging may not apply to some bids). In addition, bid developers should add new applicable entries as needed (e.g., if there is a specific certification for the industry or a particular environmental practice to add).

Evaluation CRITERIA:

The Evaluation Criteria tab is a sample evaluation criteria template to use for the bid evaluation criteria document for the Environmental Practices Form, and should be updated to match what the sourcing team includes in the form. The Sourcing Team should identify how many evaluation points to provide this form in addition to the specific points allocation (e.g., if 10 points are allocated to the form, how will the points allocation be evaluated for each answer).

Environmental Practices Form

[contract #] Environmental Proactices Form
The primary purpose of this form is to encourage bidders to communicate existing business practices that foster a sustainable approach to conducting their operations. This may include one that has a reduced impact on the environment or public health, such as, but not limited to, creating less waste by using less packaging; eliminating the use substances and/or materials that are considered toxic; using alternative fuel vehicles in the delivery fleet for the purpose of reducing air pollution and greenhouse gas emissions, or setting environmental policies and measuring progress.

The Commonwealth considers such initiatives part of the “best value” of procurement. Bidders have an opportunity to receive additional points in the evaluation phase of the RFR for the environmental initiatives they have identified. However, bidders must provide documentation to substantiate answers. If reference is made to an attached bid document, or URL, list reference in Column C.
