About this file
This document is an Equal Employment Opportunity (EEO) Staffing Plan form (M/WBE 101) required for submission with bids or proposals to the State of New York for a Construction Manager as Advisor position supporting the Electron-Ion Collider (EIC) Infrastructure project at Brookhaven National Laboratory. The form must be completed by all offerors and subcontractors and submitted as part of the bid package. Offerors are required to report either the workforce to be utilized specifically on the state contract or their total workforce, broken down by EEO job categories including Officials/Administrators, Professionals, Technicians, Sales Workers, Office/Clerical, Craft Workers, Laborers, Service Workers, and Temporary/Apprentices. The form requires detailed demographic data on all employees assigned to the work, organized by gender (male and female) and race/ethnic identification categories (White, Black, Hispanic, Asian & Pacific Islander, and Native American).
The staffing plan must also identify disabled individuals and veterans, specifically Vietnam Era Veterans, within the proposed workforce. Employees must be counted in only one race/ethnic category, which is determined by how they appear, identify, or are regarded in the community. Completed forms must include the name, title, telephone number, email address, and signature of the person preparing the report, along with the date of submission. The form serves as a compliance mechanism for contractor and supplier diversity requirements under the State of New York's procurement process and applies equally to prime contractors and all identified subcontractors.
