EMS and Prevention Billing Services
1 Introduction
1.1 Summary
The City of Santa Monica Fire Department is requesting proposals from qualified organizations to provide third party billing for services. This includes but is not limited to the billing of services such as Emergency Medical Services (EMS) Transport and related expenses, Fire Prevention billing including but not limited to: Fire and Life Safety Inspections, Annual Fire Permits, Fire Safety Officer services, Special Event Permits and Vehicle Accident cost recovery billing.
1.2 Background
SMFD is currently utilizing First Due Records Management software to document performance of inspections from the field on a tablet. Billing for EMS transports will be derived from the ePCR for all patient transports and billing for inspections would be based on the City approved fee schedule and data from the First Due inspection program (from a billing export report provided to the proposer by the City on a recurring basis) on recent inspections performed.
The proposer must indicate what information is needed from the City (i.e., billing contacts, billing amount, etc.) to administer billing for the above programs. Additionally, the proposer must identify the cost to bill which may be added onto the Fire Prevention’s customer invoice; consideration should be given to the invoice amount.
The proposer shall also provide an interface to First Due indicating when a bill is paid and at what amount to confirm full reconciliation within First Due. The proposer shall offer a billing analytics portal including, but not limited to, total inspections, inspection by individual, inspection revenue collection rate and totals, total revenue collected YTD and total anticipated revenue based on inspection inventory.
1.3 Contact Information
Any inquiries or requests regarding this procurement should be submitted via OpenGov. Other City officers, agents, employees or representatives do not have authority to respond on behalf of the City. Contact with unauthorized City personnel during the selection process may result in disqualification.

3 Scope of Work
3.1 Overview
The Santa Monica Fire Department is an all-hazards Type 1 organization. SMFD runs approximately 17,000 calls for service annually from five stations with seven front line advanced life support (ALS) capable engines, one ALS assessment staffed ladder truck, six BLS ambulances, one Advanced Provider Unit, one Hazardous Materials Unit, one cross-staff Urban Search and Rescue Unit, and one Battalion Chief. The Fire Suppression and Rescue (Operations) Division provides daily staffing of 34 firefighters to protect the City, with one Battalion Chief overseeing the on-duty crews. The City provides patient transportation services and currently transports approximately 8,500 patients per year.
EMS and Prevention Billing Services
The successful proposer shall be expected to provide general EMS billing services for all dispatch requests including emergency and non-emergency responses. These responses may include but are not limited to sick and ill persons, vehicle accidents, rescues, Interfacility Transports (IFT), Mobile Integrated Health Care Services, and any other EMS incidents that may arise.
The Fire Prevention Division is responsible for developing and implementing programs and policies that prevent or reduce the magnitude of emergency occurrences such as loss of property and life, personal injury, or environmental damage. In addition, this division reviews permits, reviews plans, and oversees the proper installation of fire sprinkler, fire alarm, and fire suppression systems. Fire inspectors conduct inspections to ensure that current codes and standards are followed. A hazardous materials team provides inspections for regulatory programs involving storage tanks and dangerous materials. The department currently performs electronic fire inspections; however, the billing for 7,500 inspections and plan checks are performed manually through the City’s utility billing system.
3.2 Objectives
The two primary objectives of this request for Request for Proposal (RFP) are to:
1) Seek a proposer to provide EMS billing for all emergency and non-emergency transports, including treatment in place and mobile integrated healthcare
2) Seek a proposer to provide a range of billing for fire inspections and other fire prevention services as per the City’s adopted annual fee schedule. The successful proposer shall have experience in similar types of services.

3.3.7 Cost recovery methodology and track record on cost recovery percentages.
3.3.8 Information and City involvement for the proposer to provide the above services.
2.3.9 Billing implementation process and proposed timeline.
Proposers shall be responsible for modifying their billing systems to capture all necessary data generated from these programs as an electronic file and provide a description of the electronic files needed to receive data transmitted from the City of Santa Monica. Proposers shall work with the City of Santa Monica and both their electronic patient care report (ePCR) vendor and fire inspection vendor to identify data input gaps, errors, or other data needs, and describe a means of obtaining solutions to these problems. 3.5 Service Level Agreement Requirements
3.5.1 Timely Response Requirement
EMS and Prevention Billing Services
The Contractor shall respond to all written or electronic communications from the City, including requests for information, data discrepancies, account inquiries, and reconciliation questions, within forty-eight (48) hours (two business days). A “response” shall consist of an acknowledgment of receipt and an estimated timeframe for completion. Failure to acknowledge or respond within the Service Level Agreement window may constitute non-performance.
3.5.2 Frequent Reconciliation
The Contractor shall conduct reconciliation of EMS transport data and Fire Prevention inspection data on a monthly basis or more frequently if preferred by the Contractor, but in no event less frequently than once per month to ensure billing completeness and data integrity. This reconciliation shall:
A. Compare data transmitted from the City’s records management and inspection systems with billing outputs.
B. Identify all records not billed, partially billed, or returned due to missing or incomplete information.
C. Provide the City a report explaining variances, unbilled items, and proposed resolutions.
3.5.3 EMS Billing Timeliness Requirements
The Contractor shall be fully responsible for complying with all insurance payer timely-filing requirements for EMS billing. Each insurance provider establishes its own filing deadlines, which may vary significantly and are subject to change. The City does not maintain or track these individual deadlines. It is therefore the Contractor’s responsibility to ensure that all claims are submitted as soon as practicable following receipt of complete documentation, and in all cases in sufficient time to meet the applicable payer’s timely-filing requirements. Failure to submit timely claims directly exposes the City to full loss of reimbursement.
3.5.4 Prevention Billing Timeliness Requirements
The Contractor shall ensure complete and timely billing of all Fire Prevention inspections, plan reviews, and related fees. Missed or delayed billings may result in lost revenue, regulatory non-compliance, and inability to reconcile the City’s inspection program.
3.5.5 Liquidated Damages
Because delayed communication or billing can result in the City’s inability to meet insurer timely-filing deadlines or to recover Fire Prevention fees, the final contract may include liquidated damages for:
A. Failure to meet 48-hour response requirements B. Failure to conduct monthly reconciliations C. Failure to bill EMS transports within insurer timeframes D. Failure to bill Fire Prevention inspections within required windows
The specific dollar amounts for liquidated damages will be negotiated during contracting and are not part of this RFP.
3.5.6 Failure to Meet Service Level Agreement Requirements
EMS and Prevention Billing Services
Repeated or material failure to meet Service Level Agreement requirements may result in withholding of payments, or other remedies to be defined in the contract.
3.5.7 Payment Posting Timeliness
When the City receives a payment directly from a customer for EMS billing or Fire Prevention services, the City will notify the Contractor of the payment. The Contractor shall post the payment to the customer’s account within forty-eight (48) hours (two business days) of receiving notice from the City. Failure to timely post payments may result in reconciliation discrepancies and financial reporting inaccuracies.

CITY OF SANTA MONICA OAKS INITIATIVE NOTICE
NOTICE TO APPLICANTS, BIDDERS, PROPOSERS AND OTHERS SEEKING
DISCRETIONARY PERMITS, CONTRACTS, OR OTHER BENEFITS FROM THE CITY
OF SANTA MONICA
EMS and Prevention Billing Services
Santa Monica’s voters adopted a City Charter amendment commonly known as the Oaks Initiative. The Oaks Initiative requires the City to provide this notice and information about the Initiative’s requirements. You may obtain a full copy of the Initiative’s text from the City Clerk.
This information is required by City Charter Article XXII—Taxpayer Protection. It prohibits a public official from receiving, and a person or entity from conferring, specified personal benefits or campaign advantages from a person or entity after the official votes, or otherwise takes official action, to award a “public benefit” to that person or entity. The prohibition applies within and outside of the geographical boundaries of Santa Monica.
All persons or entities applying or receiving public benefits from the City of Santa Monica shall provide the names of trustees, directors, partners, and officers, and names of persons with more than a 10% equity, participation or revenue interest. An exception exists for persons serving in those capacities as volunteers, without compensation, for organizations exempt from income taxes under Section 501(c)(3), (4), or (6), of the Internal Revenue Code. However, this exception does not apply if the organization is a political committee or controls political committees. Examples of a “public benefit” include public contracts to provide goods or services worth more than $25,000 or a land use approval worth more than $25,000 over a 12-month period.
In order to facilitate compliance with the requirements of the Oaks Initiative, the City compiles and maintains certain information. That information includes the name of any person or persons who is seeking a “public benefit.” If the “public benefit” is sought by an entity, rather than an individual person, the information includes the name of every person who is: (a) trustee, (b) director, (c) partner, (d) officer, or has (e) more than a ten percent interest in the entity. Therefore, if you are seeking a “public benefit” covered by the Oaks Initiative, you must supply that information on the Oaks Initiative Disclosure Form. This information must be updated and supplied every 12 months.
