About this file

This document is an Emergency Purchase Statement issued by the Illinois Department of Public Health (IDPH) through the Chief Procurement Office for General Services. The emergency procurement is for qualified nursing staff to inspect nine long-term care facilities that have fallen into financial or physical distress. The vendor will be responsible for noting improvements with regulatory compliance, verifying continued compliance and maintenance with regulations, observing facility staff, reviewing records, intervening on behalf of residents, meeting with residents, and determining if the facilities are failing to meet the direct care needs of residents. The expected term of the contract is from April 22, 2024, to July 21, 2024.

The expected expenditure for this emergency procurement is $96,750, to be funded through federal funds. The agency has stated that best efforts were made to include contractors certified under the Business Enterprise Program in the emergency procurement process. An existing state contract was not available to meet the immediate need. The vendor selected, Certisurv, LLC, has provided similar services to the agency in the past and has the necessary staff and capabilities to respond to the emergency requirement.
