About this file
This document is the Energy & Environment Cabinet Respirator Medical Evaluation Questionnaire, which is an attachment related to a state or local government contract opportunity. The questionnaire collects mandatory information from employees who have been selected to use any type of respirator, including contact details, respirator type, medical history, and cardiovascular and lung-related conditions. The document contains sections with yes/no questions covering topics such as smoking, allergies, lung problems, heart conditions, medication, and prior respirator use problems. Additional questions are included for employees using full-facepiece respirators or self-contained breathing apparatuses. This questionnaire appears to be a required attachment or document for the state or local government contract opportunity.
The document does not provide details on the specific contract, such as the products or services requested, project details, response dates, award information, or contract term. It is focused solely on the respirator medical evaluation questionnaire requirements for the selected employees who will use respirators under this contract.
