SCOPE OF OBJECTIVES (SOO)
August 19, 2014 Project Title Corporate Fitness Center Agreement Project Background & Description The Department of Education (ED) has a requirement to enter into a one-year Corporate Fitness Center agreement with one fitness center that can serve multiple locations while offering a discounted membership rate to employees located throughout ED’s Boston, New York, Philadelphia, and Washington DC offices.
Project Scope of Objectives General Requirements
· Fitness Center shall provide optional prices that reflect a $5 and $10 monthly “buy-down” capitation rate to be paid by ED and would enable employees at the Boston, New York, Philadelphia, and Washington, DC offices to receive a discounted membership rate off of the normal fitness center cost.
· First rates (Option 1) shall be based on 5% of the total employee population for the following locations: Boston (58), New York (107), Philadelphia (84), and Washington DC (3,216).
· Second rates (Option 2) shall be based on 10% of the total employee population for the following locations: Boston (58), New York (107), and Philadelphia (84).
· ED shall not be required to pay additional funds if more or less than 5% of the employee population joins the program for Option 1 or 10% for Option 2. Any employee joining over the 5% or 10% population threshold shall receive the same discounted rate. ED will decide on which Option it plans to exercise under the Corporate Fitness Agreement.
· Rates shall reflect options for employees and family members to choose either a “one club” or “multiple club” membership. All rates shall be recorded by the fitness center using the templates shown as Attachments A, B, C, and D in this SOO.
· Family members shall be allowed to participate under the program at the same discounted rate.
· Fitness Center shall allow any ED employees who are current members to take advantage of the fitness buy-down rate.
· Employees shall enroll directly with the fitness center.
Amenity Requirements
· Fitness Center shall offer, at a minimum, the following amenities for ED employees and family members: Group Fitness Classes; Cardio Equipment; Free Weights; Weight Equipment and Nutritional Resources. The Fitness Center shall use the template shown as Attachment E to identify which of the amenities would be available to ED employees and family members.
Payment Requirements
· The chosen fitness center will be offered a purchase order contract to begin the agreement. The contract shall reflect the agreement period and the portion of the membership cost ED shall pay to receive the buy-down rate. After receiving the purchase order, the fitness center shall email an invoice to Jeanie.Banks@ed.gov to include the purchase order number, invoice number, and invoice date. ED will issue payment to the fitness center via Electronic Funds Transfer (EFT) within 30 days after receiving the invoice.
· Employees and family members shall pay their portion of the membership agreement directly to the fitness center using the payment methods that are available at the fitness center.
Reporting Requirements
· Fitness center shall provide a monthly membership report reflecting the employees and family members that actual use the gym throughout the agreement period. The report shall include, at a minimum, information such as the following: First Name, Last Name, Sign Date, Membership Type, Membership Cost, Main Club Location, and Termination Date. The report shall be emailed to OMMSSupportServices@ed.gov.
Other Requirements
· Fitness Center must have an active registration in the System for Award Management (SAM) www.sam.gov in order for the government to conduct business with the company.
