About this file
This document is an Affirmation and Disclosure Form for a state contract that requires contractors to affirm compliance with Executive Orders regarding offshore services and data location restrictions. The form mandates that contractors disclose the primary business location, service performance locations, and data storage locations for both the primary contractor and any potential subcontractors. Contractors must explicitly state where services will be performed and where state data will be stored, with a clear warning that failure to provide this information may result in contract non-award.
The form emphasizes strict requirements around geographic service delivery, particularly prohibiting performance of services or data storage in offshore locations, especially those associated with Russian institutions or companies. Contractors are obligated to immediately disclose any changes in service or data location before, during, and after contract execution, with the state retaining the right to immediately terminate the contract if unauthorized location shifts occur. The document includes signature lines for official acknowledgment, requiring the contractor to confirm understanding that this disclosure is an integral part of any potential state contract.
