About this file

This document is an Affirmation and Disclosure Form used by state government contractors to confirm compliance with specific executive orders regarding offshore services and data location. The form requires contractors to disclose the principal business location, service performance locations, and state data storage locations for both the primary contractor and any potential subcontractors. Contractors must explicitly affirm that they are not performing services, locating state data, or purchasing from Russian institutions or companies.

The form includes critical legal obligations for contractors, specifically mandating immediate disclosure to the state of any changes in service or data location before, during, and after contract execution. The state retains the right to immediately terminate the contract if services are performed outside the United States without a signed waiver. By executing this form, the contractor acknowledges understanding that this document is incorporated into any potential state contract and agrees to ongoing transparency about service and data locations.
