About this file

This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the purchase of TENA 3-in-1 Cleansing Cream (or equal) for the Fiscal Year 2023 (July 2022 - June 2023). The RFQ specifies the required product features, including a 3-in-1 formula that cleans, moisturizes, and protects without the need for rinsing or towel drying. The estimated order quantity is 2,400 each (240 cases) based on current consumption, with actual quantities ordered subject to the facility's needs and availability of funds. Vendors are required to provide brand, model number, bottle size, and case quantity details in their quote.

The RFQ includes delivery requirements, warranty terms, and COVID-19 protection protocols. Vendors must hold their quoted prices valid for 90 days and include shipping and handling charges. The agency reserves the right to reject all offers, award by item or in total, and receive the lowest price in the event of a decrease. The winning vendor will be selected based on the lowest total bid for all products.
