About this file
This document is a Request for Quote (RFQ) from the Illinois Department of Veterans Affairs (IDVA) for the purchase of Aquacel brand wound dressings for the fiscal year 2023, from July 2022 to June 2023. The RFQ specifies five different types of Aquacel dressings, including foam non-adhesive squares, foam adhesive squares with borders, foam heel silicone adhesive dressings with borders, and foam sacral silicone adhesive dressings with borders. The requested quantities range from 180 to 710 boxes per item, with each box containing 10 dressings. The RFQ states that the dressings must meet specific requirements, including being adhesive, skin-friendly, and having silicone borders. Vendors must submit their quotes by the due date, and the award will be based on the lowest total bid for all products.
The RFQ outlines pricing and delivery terms, including that all prices must include shipping and handling charges, and that no price increases will be accepted. The agency reserves the right to reject all offers, award by item or group, and waive minor defects. The vendor must agree to partial shipments in case of delays in the total order. The RFQ also includes COVID-19 protection requirements, such as the use of face coverings and vaccination or testing for certain personnel.
