RFP changes
1. Add the following at the end of Evaluation Factor III, page A-10 of the Solicitation:
Submit documentation of the Workmen’s Compensation Experience Modifier Rates only for the prime contractor.
2. Add Price Form 02B after page A-35 of the Solicitation.
Questions / Clarifications The following questions and issues were submitted via email at eflhd.contracts@dot.gov. The Government response or clarification is provided. Due to the repetitive nature of several questions and issues, the wording of the questions may have been revised and some questions have not been included. The Government’s direct response to the sender may also have been updated below for better clarity.
1. Tabulation of Quantities Item 55706-2000, Structural Timber and Lumber, Treated (Pile Bent Cap Replacement) is a lump sum item. We are not sure what the quantity of cap replacement should be for the Option B Bid. Do we include all non-concrete caps (60 each) in our replacement quantity?
Response: The quantity of three caps was assumed to require replacement based on previous bridge inspection- this number is what the contractor should base their bid on.
The number to be replaced may change as the design of the project will progress.

2. Tabulation of Quantities Item 55706-2000, Structural Timber and Lumber, Treated (Pile Bent Cap Corbels is a lump sum item. We are not sure of the scope of work for this item. Do we remove existing and install new corbels at the concrete caps (6 each)? Please clarify.
Response: No, corbels will only be installed where this modification has not already been made. There are five locations.
3. Does the Government have any information as to any possibility of hazardous material being found on the existing bridge? (concern about lead paint on steel span).
Response: There is no knowledge of any hazardous materials present on the bridge (including lead paint). The steel paint was analyzed for the last bridge inspection and no lead was found.
4. Page A-12 – Pricing: Indicates that the Contractor is to use Schedule A to price the project.
We have been unable to find a Schedule A. Is Price Form 02A Schedule A?
Response: Yes, Base Option is Schedule A – and Price Form 02A should be used to bid Schedule A.
5. Price Form 02A appears to be in a spreadsheet format. Is Price Form 02a available to the Contractor in a spreadsheet format? If not then is a blank Price Form 02A available?
Response: The excel file for this form will be provided with Amendment 1.
6. Price Form Number 02B – Option B Bid Item Price Proposal is a lump sum item indicating the Contractors estimate for Bridge Rehabilitation Work. Included in Appendix F – Field Trip Report from the 30% Design Field Review is a Tabulation of Quantities (Sheet C1). Evaluation Factor III talks about a testing agency and or third party inspector that is required. What is the scope of work included in Option B? Is it only Tabulation of Quantities as listed on C1? Are we to include a full QA/QC Program?
Response: The pay items and quantities shown on sheet C1 of Appendix C – Preliminary Plans of the Solicitation are the work that the Contractor needs to bid on under Option B.
Price Form 02B for Option B- will be added to the RFP under Amendment 1. The Contractor is to include a full QA/QC Program under pay item 15401-0000.
7. Sheet C1 Item 55706-2000 indicates the FRP Pile Repair with Cathodic Protection. In order to provide Option B pricing can the government give a description or details as to what the system might look like?
Response: The work is shown on R sheets of Appendix C – Preliminary Plans of the Solicitation- see sheet R07 and sheet R12. The government will develop written specifications for the work later during the design process.
8. Evaluation Factor III page A-10 - EMR Rate. As a Contractor we will submit our EMR for the last three years. Are we to include proposed subcontractors with our proposal?
Response: No, EMR for the prime contractor only is required. This will be clarified under Amendment 1.
