About this file

This is a pricing matrix schedule for the Orphan Well New Haven Section 3 plugging project administered by the State of Michigan, specifically for three wells located in Gratiot County. The project scope encompasses comprehensive well plugging services including site supervision, labor, equipment rental, specialized tools, materials, and site restoration. The pricing matrix contains 68 line items covering contractor personnel (site supervisors, laborers), heavy equipment (derrick rigs, dozers, excavators, backhoes), specialized well-plugging equipment (BOP rentals, tubing, casing, wireline services), drilling fluids and additives, cementing services, waste disposal, and site preparation and restoration activities. Bidders are required to submit unit pricing for all line items regardless of anticipated use, as these rated items may be invoked if scope changes occur or amendments become necessary. The contract requires submission of a detailed project timeline based on 10-hour working days.

Pricing for all items on the matrix is currently listed at $0.00, indicating this is a blank template awaiting vendor quotes. Bidders must provide unit prices for every line item and calculate a total project cost. Notably, overhead percentages cannot be applied to hourly or daily rates already listed on the schedule when invoicing, though materials not specifically listed may be invoiced at cost plus the overhead percentage with prior approval from the EGLE Project Manager. All specialized equipment, rentals, supplies, and materials require advance approval from the project manager before mobilization or use. Double-billing is prohibited, such as charging separately for both laborer rates and rig crew rates for the same personnel, and rated items used to complete lump-sum work activities cannot be invoiced separately.

NOTES:
1. You must submit a project timeline as outlined in DS document. Please assume 10-hour days.
2. Provide Quantity and Price/Unit for all items listed in the Bid Table, even if the item is not likely to be used for the project. We call this 'Rated Items' and have requested the unit pricing in the event
the Scope of Work changes, a change order (aka - Amendment) is necessary, or we move to a time and materials billing.
3. Bidder cannot mobilize, use, or charge for items in the pricing matrix without prior approval from EGLE Project Manager.
4. If a ‘rated item’ is used to complete a ‘lump sum’ item work activity included in the original scope of work, it cannot be invoiced separately.
For example, if you utilize an ‘excavator’ to complete concrete pad removal (‘Other Infrastructure Removal’ bid item), you cannot charge
for the excavator or operators hours as it should have been included as part of the lump sum price for ‘Other Infrastructure Removal’
5. Only 1 Contractor Site Supervisor can be invoiced per day of onsite work.
6. Rental and/or purchase of any specialized equipment, supplies, services, or materials must have prior approval from EGLE Project Manager.
7. Overhead % cannot be added to the hourly or daily rates of items already included on Schedule B when invoicing.
8. If a material, such as 'casing' is needed to properly plug the well and is not listed on this pricing matrix, it will be invoiced at cost plus the 'Overhead %'
9. Laborer includes but is not limited to: Roustabout, Heavy Equipment Operator, Tool Hand, and other similar positions. The Laborer rate will also be used by the rig crew when they are completing work activities not assoicated
with the rig, including but not limited to: laying mats, moving equipment, loading/offloading equipment and supplies or other similar general labor duties. Rig crew cannot be invoiced at the Laborer and 'Rig Crew'
line items at the same time, as that would be double-billing.
