About this file

This is a Performance Work Statement (PWS) for full-service preventative maintenance and repair services on government-owned GE anesthesia machines across three Veterans Affairs Medical Centers within VISN 4. The contract is a five-year Blanket Purchase Agreement (BPA) running from August 1, 2026 through July 31, 2031, covering 36 anesthesia machines and associated accessories located at Altoona VAMC (13 machines), Erie VAMC (4 machines), and Pittsburgh VAMC (19 machines). Equipment models include CARESTATION 650, TEC 850, AESTIVA/5 MRI, AVANCE CS2, and AISYS CS2 units.

The contractor must provide all labor, materials, parts, and transportation to deliver preventative maintenance, calibration inspections, emergency repairs, and 24/7 technical telephone support. Services include reviewing operating system diagnostics, calibrating and cleaning equipment, replacing worn or defective parts with new GE parts only, and maintaining software updates. Preventative maintenance will be scheduled with facility Biomedical Engineering departments within 30 days of award. Routine service calls require 24-hour response time during normal working hours (Monday-Friday, 8:00 a.m. to 4:30 p.m., excluding federal holidays), while emergency calls require six-hour response time. Contractor personnel must be GE OEM-trained and certified, comply with VA security and infection control policies, complete annual cyber security and privacy training, and provide detailed service reports signed by facility representatives. The BPA includes a Quality Assurance Surveillance Plan requiring 100% observation of preventative maintenance tasks. Pricing is to be determined (TBD) with invoices submitted monthly in arrears, structured as separate line items for each facility for each contract year.

6. RECORDS MANAGEMENT Verbiage from VA Handbook 6500.6, Appendix C, 5(h)

6.1 Bio-Medical devices and other equipment or systems containing media (hard drives, optical disks, etc.) with VA sensitive information must not be returned to the vendor at the end of lease, for trade-in, or other purposes. The options are:

· Vendor must accept the system without the drive;

· VA’s initial medical device purchase includes a spare drive which must be installed in place of the original drive at time of turn-in; or

· VA must reimburse the company for media at a reasonable open market replacement cost at time of purchase.

· Due to the highly specialized and sometimes proprietary hardware and software associated with medical equipment/systems, if it is not possible for the VA to retain the hard drive, then;

· The equipment vendor must have an existing BAA if the device being traded in has sensitive information stored on it and hard drive(s) from the system are being returned physically intact; and

· Any fixed hard drive on the device must be non-destructively sanitized to the greatest extent possible without negatively impacting system operation. Selective clearing down to patient data folder level is recommended using VA approved and validated overwriting technologies/methods/tools. Applicable media sanitization specifications need to be preapproved and described in the purchase order or BPA.

· A statement needs to be signed by the Director (System Owner) that states that the drive could not be removed and that (a) and (b) controls above are in place and completed. The ISO needs to maintain the documentation.

6.2 Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.

6.3 Contractor shall treat all deliverables under the BPA as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.

6.4 Contractor shall not create or maintain any records that are not specifically tied to or authorized by the BPA using Government IT equipment and/or Government records.

6.5 Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.

6.6 Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the BPA.

6.7 The Government Agency owns the rights to all data/records produced as part of this BPA.

6.8 The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this BPA. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.

6.9 Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].

6.10 No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.

6.11 Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this BPA requiring the disclosure of information, documentary material and/or records generated under, or relating to, this BPA. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
