The following additional clauses will apply: ADDENDUM 52.212-1 Addendum to 52.212-1
I. GENERAL INSTRUCTIONS:
A. These instructions provide general guidance for preparing proposals, as well as specific instructions on the format and content of the proposal. Throughout this section, the term "offeror" includes joint venture partners. This is a continuation of the solicitation provision FAR 52.212-1.
B. Current Federal and Defense Regulations require the use of the System for Award Management (SAM) and Wide-Area Work Flow (WAWF) Invoicing System. A vendor must be registered in these systems prior to award of the contract otherwise the offeror may be considered nonresponsive if the attainment of these registrations delays award of the contract.
C. For consideration of award, offerors shall submit one (1) complete copy of the proposal package with all required clause fill-ins and pricing information by the closing date and time of the solicitation.
D. Send only unclassified written proposals (electronic is acceptable).
E. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
F. The proposal response shall consist of a copy of two (2) separate parts; Part I - Price Proposal and Part II - Past Performance Proposal.
G. Information. Your electronic proposal shall reflect the following information on the subject line:

(3) Company POC & phone number
H. Submit one (1) copy of your proposal in accordance with this Proposal Preparation Instructions, either emailed to the person(s) specified in subparagraph (I) or uploaded to DoD SAFE (safe.apps.mil):

(2) PART II - Past Performance Proposal
I. Delivery Addresses:
(1) Proposals will be emailed to: nanette.damon@us.af.mil, sean.holder.1@us.af.mil and nicole.hall.7@us.af.mil. Offeror may request verification of receipt.
J. Bidders Library. All solicitation documentation, including the Performance Work Statement (PWS) and attachments, are available on the betaSAM website: www.beta.sam.gov. Any other documents related to the solicitation not posted to the betaSAM website will be made available on an "as needed" basis upon request to the Contracting Officer. All the documents requested will be posted on betaSAM or a notification will be posted allowing all offerors access to the same information.
K. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:
(1) Requirements of the RFP (CLINs, PWS and Wage Determination) to include all applicable Government standards and regulations.
(2) Formatting shall be as follows:
(a) A page is defined as one face of a sheet of paper containing information.
(b) The Past Performance Matrix will be provided by the Government. The contractor shall not exceed three (3) pages per matrix for a total of nine (9) pages.
(c) Type size shall not be less than 12 font and in Times New Roman.
(d) Elaborate formats, bindings or color presentations are not desired or required. (e) Format proposal electronic data for readability in the Microsoft Office Suite; proposal shall be free of any virus. If files are compressed, they must be "self-extracting-archives" (no software needed to decompress files). If files contain links, the links must be intact and maintained through all revisions. Spreadsheets should be easily tractable.

II. SPECIFIC PROPOSAL INSTRUCTIONS:
A. Volume I - PRICE PROPOSAL
(1) The offeror shall submit an overall firm-fixed price proposal IAW the Collective Bargaining Agreement (CBA) that is provided by the Government.
(2) Insert proposed unit and extended prices on the DD Form 1449 for all Contract Line Item Numbers (CLINs).
(3) In providing complete pricing information as requested, the offeror agrees to all contract terms and conditions as written in the solicitation with all attachments.
B. Volume II - PAST PERFORMANCE
(1) The Government may review up to three (3) total contract's past performance submitted by the offeror. Any submission in excess of the limit will not be considered and only the first three (3) contracts listed will be evaluated. One (1) of past performances must show the contractor as a prime.
(2) Past performance for contracts where the offeror performed as the prime contractor and/or subcontractor may be considered.
(3) The offeror shall provide no more than three (3) of the most recent and relevant contracts performed for Government Agencies and/or commercial customers.
(4) To be recent, the contract (or subcontract) must have been performed within the past five (5) years from the date of issuance of this solicitation (calendar years 2015-2020) for a minimum of a six (6) month period. Past performance outside this given timeframe will not be evaluated.
(5) There are four (4) levels of relevancy. To be considered relevant, the contract (or subcontract) must have had a customer headcount of a minimum of 17,000 customers monthly.
(6) The Government requires the offeror to provide a Point of Contact (POC) for each contract reference. The POC for each reference may be contacted by the Government for additional information if required by regular mail, email, or phone IAW the Past Performance Matrix (Tab 2).
(7) The Government will evaluate the offeror on the quality of past performance under recent and relevant contracts (or subcontracts). The offeror shall provide, a completed Past Performance Matrix (Tab 2).The following items are identified for each reference provided:
(a) Contractor POC name and Position/Duty Title
(b) Company/Organization name, email & telephone number of the Contractor POC
(c) Contract Number
(d) Prime/Subcontractor or Key Personnel Designation
(e) Project Title
(f) Brief Description of Work
(g) Location of Performance
(h) Period of Performance
(i) Contract Dollar Value (Annual and Aggregate)
(j) Verified, up-to-date name, email & telephone number of the Contracting Officer
(k) Verified, up-to-date name, email & telephone number of the Contracting Officer Representative
(8) Offerors are reminded that both independent data and data in proposals may be used by the Government to evaluate past performance.

Addendum to 52.212-2
I. SOURCE SELECTION:
A. Basis for Contract Award
(1) This is a Price-Performance Trade-off (PPT) source selection conducted in accordance with Federal Acquisition Regulation (FAR) Part 15, Contracting by Negotiation, as supplemented by the Defense Federal Acquisition Regulation Supplement (DFARS), the Air Force Federal Acquisition Regulation Supplement (AFFARS), DoD Source Selection Procedures, Appendix A, dated 31 March 2016 and Air Force Mandatory Procedures 5315.3 2019 Edition. These regulations are available electronically at the FARSite, http://www.acquisition.gov.
(2) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
(3) Solicitation Requirements, Terms and Conditions. Offerors are required to meet all solicitation requirements, such as terms and conditions, representations and certifications, evaluation factor requirements, and be responsible in accordance with FAR Part 9. Failure to comply with the terms and conditions of the solicitation may result in the offeror being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and must provide complete supporting rationale. The Government reserves the right to determine any such exceptions unacceptable.
B. Evaluation Methodology
(1) The Government will evaluate proposals using two Factors: (1) Past Performance, and (2) Price. Past Performance is significantly more important than price.
(2) The Government reserves the right to award without discussions, open discussions, or to establish a competitive range IAW FAR 15.306(c). If discussions are necessary they will be conducted IAW FAR 52.212-1(g).
C. Evaluation Factors
(1) Each proposal evaluation includes:
(a) Factor 1: Past Performance
(b) Factor 2: Price
D. Factor 1 – Past Performance
(1) The Government will conduct a past performance evaluation that examines an offeror's recent and relevant past performance record to assess the government’s confidence in the offeror’s ability to perform as proposed. There are three aspects to the past performance evaluation: recency, relevancy and quality.
(2) In order to meet the criteria for recency, the referenced contract(s) must have been performed within the past five (5) years from the date of issuance of this solicitation (calendar years 2015-2020) for a minimum of a six (6) month period.
(3) In order to meet the criteria for relevancy, reference table below.

With respect to relevancy, past performance of greater relevancy will be considered a stronger predictor of future success and have more influence on the past performance confidence assessment than past performance of lesser relevance.
(4) The quality aspect of the past performance evaluation indicates how well the contractor performed on previous contracts. The past performance evaluation performed in support of a current source selection does not establish, create, or change the existing record and history of the offeror’s past performance on past contracts; rather, the past performance evaluation process gathers information from customers on how well the offeror performed those past contracts.
(5) Past performance, information obtained through the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and other databases, interviews with program managers and Contracting Officers, or other sources known to the Government.
(6) The Government will review this past performance information and determine the quality and usefulness as it applies to the performance confidence assessment. The past performance confidence assessment rating is based on the offeror’s overall record of recency, relevancy, and quality of performance. In conducting a performance confidence assessment, each offeror shall be assigned one of the ratings below:

E. Factor 2 – Price
(1) The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(2) To determine overall reasonableness the price proposal will demonstrate understanding of contract requirements and ability to provide contract services. To determine a fair and reasonable price adequate price competition is expected, but the Government may use any acceptable method as described in FAR Part 15.
