RFP Solicitation Template Request Rev. 6.2025
REQUEST FOR PROPOSAL (RFP)
The Ohio Department of Health (ODH) is soliciting proposals for professional services.
1. PROJECT INFORMATION:
1.1. Project Title: National Diabetes Prevention Program (National DPP) Employer Coverage
Technical Assistance
1.2. Solicitation Posting Date: 08/26/2025
1.3. Inquiry Start Date: 8/26/2025
1.4. Inquiry End Date: 09/22/2025 at 3 p.m. All questions must be submitted via Ohio Buys.
Questions received after this date will not receive a response.
1.5. Solicitation End Date: 9/30/2025 at 3 p.m. All required application components must be received by this date. Submissions must be submitted via Ohio Buys. Each application component must be clearly labeled.
1.6. Project Background:
The Diabetes Prevention and Management Program (DPMP) in the Bureau of Health
Improvement and Wellness at the Ohio Department of Health (ODH) received a cooperative agreement from the Centers for Disease Control and Prevention (CDC) to fund diabetes prevention and management activities. Ohio has a rich history of working in chronic diseases under various cooperative agreements from the Centers for Disease Control and Prevention, most recently Improving the Health of Americans Through Prevention and Management of
Diabetes and Heart Disease and Stroke (DP-18-1815).
Chronic diseases such as diabetes, heart disease, stroke, and many cancers are among the most common, costly, and preventable of all health problems in both the United States and Ohio.
Chronic disease is associated with reduced quality of life, poor health outcomes, increased healthcare needs, and higher healthcare costs. In 2020, Ohio ranked among the worst in the country for diabetes (40th) and cardiovascular deaths (38th). Ohio Vital Statistics data show that diabetes was the 8th leading cause of death in 2022, accounting for 4,263 deaths.
According to 2022 data from the Ohio Behavioral Risk Factor Surveillance System (BRFSS), Ohio adults have a significantly higher prevalence of diabetes (13.2%), heart disease (8.4%), and stroke (4.3%), compared with the U.S. prevalence. More than 1 in 3 adult Ohioans have obesity
(38.1%), high blood pressure (35.6%), and high cholesterol (35.5%), and 11.8 percent of Ohio adults have been diagnosed with prediabetes.
The burden of diabetes disproportionately impacts certain populations in Ohio. The estimated prevalence of diabetes is significantly higher among populations with the lowest income and lowest education. In addition, chronic disease mortality rates by race/ethnicity indicate that Black people have higher death rates of diabetes, heart disease, stroke, and kidney disease, compared with other racial groups. There are also marked geographic disparities in Ohio. In 2022, the diabetes and heart disease death rates in the 32-county
Appalachian region of Ohio were higher than in the non-Appalachian counties. Other groups of
Ohio adults with a significantly higher prevalence of diabetes include those with disabilities, veterans, those aged 65 and older, and those who report poor mental health. In addition, adults aged 65 and older and adults with disabilities have a disproportionally high prevalence of prediabetes.
The goal of the ODH DPMP is to support implementation of evidence-based strategies and interventions to decrease risk for type 2 diabetes among adults with prediabetes, and improve self-care practices, quality of care, and early detection of complications among people with diabetes in Ohio and among selected priority populations to achieve improved health outcomes and reduce health disparities.
1.7. Project Objective:
The objective of this project is to increase availability of the National DPP lifestyle intervention as a covered health benefit for employees and covered dependents at high risk for type 2 diabetes. To accomplish this, ODH will contract with a vendor that will provide technical assistance to employers and health plans to: 1) pilot the National DPP for their employees and/or covered beneficiaries, and 2) work toward adding the National DPP as a covered medical benefit.
The vendor will work with employers and health plans in one or more of the selected counties: Adams, Allen, Ashtabula, Athens, Clark, Coshocton, Cuyahoga, Fayette, Franklin, Gallia, Guernsey, Hamilton, Harrison, Highland, Jackson, Jefferson, Lawrence, Lorain, Lucas, Mahoning, Marion, Meigs, Montgomery, Morgan, Muskingum, Pike, Richland, Ross, Scioto, and Vinton.
