The addendum provides modifications to Attachment A Specifications and answers 45 vendor questions. Key changes include extending the delivery date from 2/28/2025 to 6/30/2025, increasing the generator requirement from 10kw to 12kw, modifying the HVAC requirements to two separate rooftop AC units, changing the handicap access from a ramp to a UVL wheelchair lift, and reducing the fresh water tank requirement from 50 to 40 gallons. The unit must include laminated walls, black roller blinds, commercial grade flooring, exam room with table and doctor's stool, medical grade refrigerator, stainless steel sinks with hot water, storage cabinetry, safety detectors, Wi-Fi capability, and various electrical and plumbing systems. Exterior requirements include two access doors, wheelchair lift, LED lighting, and a powered awning. The vendor must provide onsite training within 7 days of delivery and a minimum one-year warranty. The DOH clarified that no progress payments will be allowed, no lab area or reception area is required, and no graphics wrap should be included in the bid response.
