About this file
This document is Addendum #2 to the Invitation to Bid (ITB) DOH22-029 issued by the Florida Department of Health (DOH) for the purchase and installation of a 20-unit temperature-controlled grocery locker, including maintenance, monitoring, and wrapping services, to be provided to the DOH in Jefferson County. The addendum provides responses to questions received from prospective vendors regarding the project requirements, such as confirming the ability to recognize a funding partner's logo on the physical locker, the need to list subcontractors, and the availability of a preferred vendor list. It also clarifies that vendors are not allowed to access the DOH's property to obtain site work quotes prior to the bid submission deadline.
The ITB specifies that the contract will be awarded to the lowest responsive and responsible bidder. No additional details are provided regarding pricing terms, set-asides, incumbents, or funding requirements and sources.

# Question Answer
1. Re: Attachment 1 Exhibit 1 (last page) - In order to execute this project, our organization will leverage support from another funder, Florida Blue Foundation, for $100,000 for this project to cover other project costs such as food, delivery, food storage and marketing costs. We have already confirmed with the funder that we are anticipating a contract with the Jefferson County to support with costs related to purchase and installation of the locker. Are we able to confirm with this foundation partner that their support will be recognized on the physical locker (with prior approval for all parties of the mock-up for how the logos will appear), and at any potential ribbon-cutting event (if applicable)?
Yes, the partner's logo can appear on the physical locker in a location and size determined and approved by DOH-Jefferson.
1. Re: Attachment G (page 29) - Do we need to list the vendors (e.g., cement vendor) that our organization will work with for this project under the Subcontractors List? If so, do we need to submit multiple quotes for each subcontractor?
Yes, Subcontractors should be listed as indicated on Attachment G.
No, submission of multiple quotes are not required.
1. Does Jefferson County DOH have a preferred vendor lists for work such as concrete or electrical jobs?
No, the Department does not have a preferred vendor list.
1. Can Second Harvest access DOH's property with vendors we are getting site work quotes from prior to 3/14? For example, we want to meet with concrete vendors to get quotes on that portion of the work.
No, this is not allowed.
