About this file

This is a price form for Invitation for Bid (IFB) DOGAT30SBR issued by the Maryland Department of Transportation (MDOT) State Highway Administration (SHA) for Gate Operator Maintenance and Repair services in Dorchester County. The contract requires bidders to provide pricing for four line items: quarterly gate preventative maintenance inspections (12 per year), repair labor at straight time hourly rates for technicians performing gate repairs and maintenance (60 hours estimated), trip fees or charges for repair calls (6 per year), and repair parts and materials based on manufacturer's suggested retail price (MSRP) capped at $3,000 per occurrence. Bidders must complete the price form in its entirety without alteration and submit only the stipulated pricing information.

This procurement is designated as a Small Business Reserve contract, requiring all bidders to meet Maryland's small business certification qualifications under State Finance and Procurement Article 14-501(c). Bidders must affirm their small business status under penalty of perjury and notify the procurement officer within 30 days if their status changes during contract performance, with failure to do so resulting in contract termination. Bids should not include Maryland sales and use taxes or federal excise taxes, as SHA holds exemption certificates for both. The total bid amount is the sum of all pricing columns, and bidders must provide company information, authorized representative signature, federal tax identification number, and contact details.
