About this file
This is a bidder required form issued by the State of Wisconsin Department of Administration, State Bureau of Procurement, for UW-Milwaukee's solicitation of Audiology Equipment, Accessories, and Supplies (Solicitation Reference Number KK21-01-O). The form establishes mandatory completion requirements for all prospective bidders and serves as the official response document for this procurement opportunity. Bidders must complete all sections of the form, including bidder information, contact details, company references, and required certifications. The form does not specify item quantities, response deadlines, bidder conferences, or contract award dates within its content.
The form requires bidders to provide pricing information, which becomes public upon bid opening, while proprietary and trade secret information may be designated confidential subject to Wisconsin Statutes and the Open Records Law. Bidders may opt to participate in a piggybacking provision allowing other Wisconsin State Departments and UW System Universities to reference the resulting contract for their own purchases. The solicitation includes preferences for certified Minority Business Enterprises and Disabled Veteran Owned Businesses, each eligible for up to 5 percent preferences under Wisconsin Statutes. Bidders must certify compliance with various statutory requirements, including non-collusion attestations, non-boycott of Israel certifications, and material sourcing compliance with United States manufacturing preferences when all other factors are equal.
