About this file

This is a Bidder Required Form (DOA-3832) issued by the State of Wisconsin Department of Administration for the University of Wisconsin-Milwaukee's Service and Preventative Maintenance bid for Print Shop Equipment, solicitation reference number KK19-03-M. The form serves as a mandatory submission document that all bidders must complete in their entirety, with the exception of Section 5 if the agency designates it as not applicable. Bidders are required to provide complete company information, including contact details, mailing address for purchase orders, and designated contacts for bid-related questions, affirmative action matters, and orders and billing. Additionally, vendors must supply a minimum of four company references with similar product or service experience, including company names, addresses, contact persons, email addresses, phone numbers, and descriptions of relevant products and services provided.

The form requires bidders to designate any proprietary or confidential information they wish to protect as trade secrets under Wisconsin Statutes, though pricing information cannot remain confidential and must be released upon bid opening. Bidders must certify various compliance matters, including non-collusion in the bidding process, non-participation in boycotts of the State of Israel, compliance with all terms and conditions, and agreement to purchase materials manufactured in the United States when all other factors are equal. The form also provides opportunities for bidders to claim minority business enterprise preference, disabled veteran-owned business preference, or work center certification under Wisconsin law, each requiring appropriate state certification. All certifications must be signed by an authorized company representative with contact information provided.
