About this file
The document is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with Public Contract Code §10162, as part of the Environmental Consulting Services contract opportunity. The form requires prospective bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The disclosure form is a standard compliance document that allows bidders to transparently report any past issues that might impact their eligibility for the contract. Bidders must answer "Yes" or "No" to the disqualification question, with the option to attach explanatory documentation if they have experienced any such disqualifications. This form is part of the City of Pasadena's due diligence process for selecting an environmental consulting services provider to support the city's Safety Division with monitoring and sampling services for incidents involving asbestos, mold, lead, or indoor air quality on city properties.
