About this file

The document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, specifically related to the City of Pasadena's Bike Month 2024 and 2025 contract opportunity. The form requires prospective bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no questionnaire where bidders must indicate their compliance history, with an instruction to explain and attach documentation if the answer is "yes". This disclosure serves as a mechanism for the City of Pasadena to assess the bidding entity's past performance and potential risks before awarding the contract for planning and conducting public outreach events during Bike Month Pasadena in May 2024 and May 2025.
