About this file

The document is a Disqualification Disclosure form for the Rose Bowl Operating Company (RBOC), complying with California Public Contract Code §10162. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that allows bidders to transparently report any previous project disqualifications by checking either "Yes" or "No" and providing additional documentation if applicable. This disclosure is part of the procurement process for the Rose Bowl Operating Company's Point of Sale System project, which seeks to implement a comprehensive POS system across the Rose Bowl Campus, including the stadium, Brookside Golf Club, and surrounding grounds.
