About this file
This document is a Disqualification Disclosure form in compliance with California Public Contract Code §10162, required for a bidder participating in the Rent Stabilization Department's Marketing, Design and Communication Services contract opportunity. The form requires bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The disclosure form provides a simple binary response option (Yes or No) with an instruction to explain and attach documentation if the answer is affirmative. While the form is part of a broader procurement process for marketing and communication services, this specific document serves as a mandatory compliance and transparency mechanism to ensure the integrity of potential contractors in the government procurement process. The form is designed to help the Rent Stabilization Department evaluate the past performance and legal compliance of prospective bidders before awarding the contract.
