About this file
This document is a Disqualification Disclosure form in compliance with California Public Contract Code §10162, specifically designed for prospective bidders in state and local government contract opportunities. The form requires bidders to disclose whether they, their officers, or employees with proprietary interest have ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations.
The form provides a simple yes/no checkbox mechanism for bidders to declare their compliance history, with an option to provide detailed explanations if the answer is affirmative. By completing this document under penalty of perjury, bidders must transparently report any past disqualifications, which helps government entities assess the reliability and integrity of potential contractors. In the context of the Film and Event Monitor opportunity, this disclosure ensures that selected monitors have a clean professional record when providing oversight for city ordinance and regulation compliance.
