About this file

The document is a Disqualification Disclosure form required by the California Public Contract Code (PCC §10162) for the City of Pasadena's Traffic Counts FY2025 contract opportunity. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that allows bidders to transparently report any past incidents of disqualification by checking either "Yes" or "No". If the bidder checks "Yes", they are required to explain the circumstances and attach supporting documentation to the exhibit. This disclosure is part of the City of Pasadena's due diligence process for the traffic data collection project, which aims to ensure contractor integrity and accountability before awarding the professional services contract for traffic studies.
