About this file
This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162, applicable to prospective bidders for the CalTrans Real Property Sale involving residential property inspectors. The form requires bidders to disclose under penalty of perjury whether the bidder, any officer of the bidder, or any employee with a proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. The City is seeking qualified residential property inspectors to support its acquisition and disposition of approximately six vacant single-family or duplex properties within its real estate portfolio.
Bidders must complete this mandatory disclosure by checking either "Yes" or "No" in response to the disqualification question. Should a bidder answer affirmatively, they are required to explain the circumstances in detail and attach supporting documentation to the disclosure exhibit. This form serves as a prerequisite qualification mechanism to ensure that only eligible contractors with no prior disqualifications or safety violations participate in the procurement process.
