About this file
This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162, applicable to bidders for the Caltrans Real Property Sale: Residential Property Appraisers contract opportunity. The form requires prospective bidders to disclose, under penalty of perjury, whether the bidder, any company officer, or any employee with a proprietary interest in the bidding entity has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate yes or no in response to this mandatory questionnaire.
If a bidder answers affirmatively to the disqualification question, the form requires submission of a detailed explanation of the circumstances along with supporting documentation. This disclosure requirement applies to all prospective bidders seeking to provide residential property appraisal services to the City in connection with its acquisition and disposition of approximately six vacant single-family or duplex properties in California. Compliance with this disclosure is a prerequisite for bid submission and evaluation.
