About this file

The document is a Disqualification Disclosure form required by California Public Contract Code (PCC) §10162, associated with the City of Pasadena's Advanced Metering Infrastructure (AMI) Consulting and Program Management Services procurement. The form requires prospective bidders to disclose under penalty of perjury whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no checkbox mechanism for bidders to self-report any past disqualifications, with an option to explain circumstances and attach supporting documentation if the answer is affirmative. This disclosure is part of the City of Pasadena's due diligence process for the AMI project, which aims to select a qualified consulting firm to support the planning, implementation, and management of advanced metering infrastructure technologies across the city's infrastructure.
