About this file

The document is a Disqualification Disclosure form related to the City of Pasadena's Safe Speed Campaign, which requires prospective bidders to complete a questionnaire under California Public Contract Code §10162. The form requires bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The disclosure form is part of a consultant services solicitation for developing and implementing a motorist safety campaign and outreach program, funded by a grant from the California Office of Traffic Safety through the National Highway Traffic Safety Administration. Bidders must answer "Yes" or "No" to the disqualification question and, if applicable, provide an explanation and attach supporting documentation. This form serves as a screening mechanism to ensure the integrity and past performance of potential contractors for the pedestrian and bicyclist safety program.
