About this file

The document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, requiring prospective bidders to complete a questionnaire under penalty of perjury. The form asks bidders to disclose whether the bidder, any officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. The form provides a simple yes/no checkbox for the bidder to indicate their status, with an option to explain circumstances and attach supporting documentation if the answer is yes.

While the document is a standard compliance form, it is part of a broader procurement process that requires transparency and accountability from potential contractors. The disclosure helps government entities assess the past performance and reliability of bidders by examining their history of regulatory compliance. By mandating this disclosure under penalty of perjury, the form serves as a mechanism to ensure that only contractors with clean records are considered for government projects, thereby protecting public interests and maintaining high standards of professional conduct in government contracting.
