About this file

The document is a Disqualification Disclosure form in accordance with California Public Contract Code (PCC) §10162, intended for prospective bidders on a project related to the State Route 710 (SR 710) North Stub Historical Background study. The form requires bidders to disclose whether the bidder, any officer, or employee with proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that requires a simple yes or no response, with the option to provide additional explanation if the answer is affirmative. It is part of a larger procurement process for a historical research project involving community displacement, with the City planning to select one or more consultants to perform tasks related to documenting the historical impacts of the SR 710 North construction, which displaced approximately 4,000 residents and 1,500 homes, predominantly affecting low-income residents and people of color. The project will involve developing materials, maps, and conducting community outreach with potential translation and interpretation services.
