About this file
The document is a Disqualification Disclosure form from the City of Pasadena, California, specifically related to the City's Strategic Broadband Plan procurement process. The form, aligned with California Public Contract Code §10162, requires prospective bidders to disclose under penalty of perjury whether the bidder, any company officer, or any employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.
The form is a standard compliance document that allows bidders to transparently indicate their past project performance by checking either "Yes" or "No" to the disqualification inquiry, with an option to attach explanatory documentation if the response is affirmative. This disclosure is part of the City of Pasadena's procurement process for developing a Strategic Broadband Plan, which seeks innovative consulting services to assess broadband infrastructure, develop planning and design strategies, and explore alternative financing approaches such as public-private partnerships, grant acquisition, and resource swapping.
