About this file

This document is a Disqualification Disclosure form required under California Public Contract Code §10162, which is part of a bid submission for the ServiceNow IT Asset Management & ITSM Enhancements project. The form requires prospective bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form provides a simple yes/no checkbox mechanism for bidders to self-report any past disqualifications, with an option to attach explanatory documentation if the answer is affirmative. This disclosure is a standard compliance requirement designed to ensure transparency and accountability in the government contracting process, specifically for the California state government's Department of Information Technology (DoIT) ServiceNow implementation project focused on asset and cost management reporting enhancements.
