About this file

The document is a Disqualification Disclosure form in compliance with California Public Contract Code §10162, specifically related to the Pasadena Water and Power Department's Security Services contract opportunity. The form requires prospective bidders to disclose whether the bidder, any officer, or employee with a proprietary interest has ever been disqualified, removed, or prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation.

The form is a standard compliance document that allows bidders to indicate (by checking "Yes" or "No") whether they have any previous disqualifications. If the answer is "Yes," the bidder is required to explain the circumstances and attach supporting documentation to the exhibit. This disclosure is part of the procurement process for the Department of Water and Power Security Services contract, which seeks a vendor with a State of California Private Patrol Operator license to provide security monitoring and safety services for PWP properties.
