About this file

This document is a New Jersey state-mandated Certification of Non-Involvement in Prohibited Activities in Russia or Belarus form, issued pursuant to N.J.S.A. 52:32-60.1 and dated January 22, 2024. The certification is required for any vendor seeking to enter into or renew a contract with a state agency for goods, services, or bond purchases, and must confirm the vendor's status regarding the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list.

The form provides three certification options: (A) the vendor is not identified on the OFAC list for activities related to Russia/Belarus, (B) the vendor is identified on the list for Russia/Belarus-related activities, or (C) the vendor is identified but claims their activities are consistent with federal law. The document includes comprehensive definitions of "vendor" that encompass various business entities, governmental instrumentalities, and related organizations. Vendors must complete the form, providing their contact information, and face potential sanctions, compliance actions, damage recovery, default declaration, or suspension if they make a false certification. The form requires a signature from an authorized representative and allows for additional explanatory documentation if needed.
