About this file

This document is a Disclosure of Investment Activities in Iran form, required by New Jersey state law for entities bidding on or proposing to enter into a contract with a state or local government. The form is part of the bidding process for a City of Newark contract for Corporation and Curb Stops, Couplings and Hitches (Bid #2025-49), with electronic bids to be received until 10:15 am on Thursday, July 10, 2025. Bidders must certify whether they or their parent/subsidiary entities have any investment activities in Iran, with potential consequences including sanctions, compliance actions, damage recovery, default declaration, or debarment if found in violation.

The form requires bidders to complete three parts: Part 1 involves certifying whether the entity is on the New Jersey Department of Treasury's Chapter 25 list of entities with Iranian investments, Part 2 provides space for detailed disclosure of any such investments, and Part 3 requires a sworn certification of the information's accuracy. Signatories acknowledge that making a false statement is a criminal offense and could result in criminal prosecution and voiding of any resulting contracts. The certification also places the bidder under a continuing obligation to notify the contracting unit of any changes to the disclosed information throughout the contract period.
