About this file

The document is a "Disclosure of Investment Activities in Iran" form from the New Jersey Department of the Treasury, which is a mandatory certification for entities seeking to enter into or renew a state contract. The form requires bidders or proposers to attest, under penalty of perjury, whether they or their parent entities, subsidiaries, or affiliates are identified on the State Department of Treasury's Chapter 25 list as engaging in investment activities in Iran. Bidders must review the Chapter 25 list prior to completing the certification and have two options: certify no investment activities in Iran or provide a detailed description of any such activities.

The form outlines potential consequences for vendors found in violation, which may include imposing sanctions, seeking compliance, recovering damages, declaring the party in default, and seeking debarment or suspension. By completing the certification, the bidder acknowledges a continuing obligation to notify the contracting unit of any changes to the information provided. The document also includes a warning that making a false statement or misrepresentation is a criminal offense that could result in criminal prosecution and may void any resulting contracts. The form requires the bidder's full name, title, signature, and date to validate the certification.
