About this file

This document is a Disclosure Debarment Required Form for a government procurement opportunity. The form requires bidders to disclose any potential conflicts of interest or relationships between the bidder's organization and any City elected officials or employees, including any campaign contributions made within the past two years. Additionally, the form certifies that the bidder and its principals have not been debarred, suspended, proposed for debarment, or declared ineligible by any federal, state, or local entity, and have not been convicted of certain offenses or had contracts terminated for default within the past three years. Failure to disclose the required information or submit the completed form may result in the bid being deemed non-responsive.

The document outlines the legal requirements for the bidder's organization, including compliance with state and federal equal opportunity and non-discrimination conditions of employment, and prohibitions on engaging in business ventures or providing gifts or favors to City employees. The form must be signed by an authorized officer or agent of the bidder's organization and notarized.
