About this file
This is a Cost Proposal attachment (DISC 1264 Attachment 4) for the New York State Office of Children & Family Services (OCFS) solicitation for developmental writing and technology seminar services. The procurement seeks a qualified organization to provide web-based technology training and specialized developmental writing instruction for high school students who are blind and attending the Pre-College Program. The Pre-College Program serves legally blind students entering their senior year of high school and provides a four-week on-campus experience designed to prepare participants for college through instruction in academic, social, and independent living skills. The proposed course must incorporate instruction on both assistive technologies for blind individuals and mainstream technologies applied to traditional college writing, with students receiving individual and group writing instruction remotely while learning to use online learning tools including iOS access devices, electronic course management systems, web conferencing, and discussion forums. OCFS intends to select one organization based on specified evaluation criteria, and the contract will be awarded pursuant to Section 163 of State Finance Law as a discretionary purchase.
Applicants must complete the cost proposal by entering dollar amounts in the highlighted cells only, with all required functionality costs included in the proposal. The proposal requests pricing for three deliverables across five contract years: (1) Curriculum development/updating, Train the Trainer, and one-on-one trainer instruction including all associated travel costs; (2) Participant Administration and Program Delivery; and (3) Evaluations, Post-Program Assessments, and Reports. The total price for all deliverables combined cannot exceed $350,000 for the entire five-year contract term. All costs must be entered in currency format rounded to the nearest whole dollar, and applicants must not leave any fields blank. By signing the Cost Proposal, applicants agree that the prices quoted are binding for 365 days from the proposal due date.
