About this file

This is a Request for Quote (RFQ) issued by the State of Illinois Department of Human Services for Packard Mental Health Center (PMHC), specifically soliciting a contract for dietitian services under RFQ number 26-444DHS-MENTH-B-48526. The state seeks a licensed dietitian to provide approximately 20 hours per week of on-site services, filling in for a full-time employee absence. The dietitian's essential functions include conducting nutritional screenings, completing patient nutrition assessments, assigning risk ratings, interpreting diet prescriptions, menu planning, and directing diet preparation. The contract term will run from the last updated signature date through June 30, 2026, with the primary goal of maintaining a safe patient environment.

The vendor must possess a current dietitian nutritionist license from the Illinois Department of Financial and Professional Regulation, in compliance with the Dietitian Nutritionist Practice Act. Telehealth services are explicitly not permitted, requiring physical on-site presence at the Packard Mental Health Center. No specific pricing details are provided in the document, and the solicitation appears to be a small purchase procurement focused on acquiring specialized professional services to support the mental health facility's nutritional care requirements.
