About this file
This is a Data Item Description (DID) that establishes the format and content requirements for the Report of Shipping (Item) and Packaging Discrepancy form (SF 364).
The DID applies to contractors receiving Government materiel during performance of defense contracts. The report enables contractors to notify the Government when freight contents do not agree with shipping documents and provides the basic documents required to support adjustment of property and financial inventory accounting records and to provide information as a basis for claims. The applicable form is SF 364, with AMSC number A6284. Contractors must prepare reports using SF 364 and submit them to the U.S./Army Materiel Management agency (AMMHC-MD). Content requirements include: date of preparation (year, month, day format); report number using the contract DOD Activity Address Code followed by a four-digit number (001-9999); recipient information with name, address, ZIP code, DODAAC, or routing identifier; shipper name and address when different from recipient; invoice number and date; transportation document type (GBL, CBL, manifest, waybill); shipment number and contract/document number; administering contract office; requisitioner's number; NSN or part number with nomenclature; unit of issue; quantity shipped/billed; quantity received; unit price; total cost; discrepancy code; and action code. Item 12 (remarks) should include supplemental information regarding the discrepancy, specific details such as lot or batch numbers, manufacture date, inspector details, probable cause, and suggested corrective action. The form includes detailed discrepancy codes (conditions of material) and action codes to classify the nature and requested resolution of reported discrepancies. A sample completed form (Figure 1) and detailed instructions are provided.
